Medical home health

Skilled care after illness, injury, or surgery

Home health is a wide range of services that can be given in your home. It is often more convenient than facility-based care and is designed to help you get better, regain independence, and become as self-sufficient as possible.

Skilled nursing

Registered nurses provide wound care, medication teaching, injections, IV support when ordered, disease monitoring, and education for you and your family.

Medical social work

Social workers connect you with community resources, counseling, and care coordination when life and health feel overwhelming.

Home health aide services

Certified aides assist with personal care—bathing, dressing, grooming, and mobility—as part of a physician-ordered clinical plan.

Who we help

Support after the hospital—and for living well at home

  • Recovery after surgery, stroke, heart failure, or a hospital stay
  • Wound care, medication teaching, and chronic disease management
  • Fall prevention, strength building, and safer mobility
  • Extra help with daily activities so family caregivers can rest

A physician’s order is needed to start skilled home health. Once you are referred, we schedule a visit, talk with you about your needs, and keep your doctor updated on your progress.
